CONCENTRATION FORM INSTRUCTIONS
GENERAL INSTRUCTIONS
Every student is required to complete the Concentration Form when declaring to become an Accounting Major.
The Concentration Form is a planning tool. Its purpose is to help you create a "budget" of courses required to major in Accounting.
You must complete this form online. Once the form is completed, click on the PRINT button at the bottom of the form and print TWO copies.
This form cannot be submitted electronically (since your signature is required).
Sign and Date both copies of the form.
Meet with your advisor during regular office hours (or contact your advisor to request an appointment at an alternate time).
Your advisor will Sign and Date both copies.
One copy is for you to keep. The other copy will be kept by the advisor and will be used to approve your graduation.
IMPORTANT: Since this is a planning tool, you will probably end up making changes to the courses selected or the semester in which the courses are taken. You may make changes anytime with the advisor's approval. Unauthorized changes in the courses may prevent you from graduating.
TRANSFER STUDENTS
must also provide their advisors with a copy of their Queens College transcript. (If the transcript has not been so updated for transfer credits, a copy of the Transfer Credit Evaluation Form should be provided.)
